How do I receive my medicine at home?

Simply opt into the Connect-TED program and Wellfount will send your medications home with you upon discharge.

How do I pay my bills?

Make a check or money order payable to Wellfount Corp and mail it to 5751 W. 73rd St. Indianapolis IN 46278. Call 800-949-4979 for credit card payments. Automatic payment/ACH options are available as well, ask your Connect-TED team how to set up!

What do I do if I receive a new RX from my doctor?

Please contact Wellfount and we can assist you as to what steps you need to take.

What if my insurance changes?

Please call 800-949-4979 and follow the prompts for billing to update your insurance information. You can also mail a copy of the front and back of your insurance card to 5751 W. 73rd St. Indianapolis IN 46278 or fax to 1-888-TED-RXCT (1-888-833-7928).

What if my doctor changes?

Please contact the Connect-TED team immediately to let us know of any changes to your doctors, address, phone number or insurance. Additionally you can fill out the personal information change section on the back of your statement and mail it to 5751 W. 73rd St. Indianapolis IN 46278.

What I develop a new medical condition?

Contact Wellfount and we will reach out to your doctor to see how best to proceed.
